Print your paperwork at home, fill it in at your own pace, and bring the pages with you. It saves you sitting in our waiting room with a clipboard, and it means you can look up your medication names and insurance details somewhere more comfortable than a dental office.

A few practical notes
- Print and fill these in by hand — there is nothing to submit online.
- This paperwork asks for your medical history, your insurance details, and your Social Security number. Please do not email, text, or message any of it to us — bring the filled-in paper with you.
- No printer? No problem. Come about ten minutes early and we will hand you a copy.
- If anything on a form does not make sense, leave it and we will go through it together.
